OQ-06: Is female pattern hair loss around menopause driven by hormones, and does anything change its course?

Status: open. Status as of . Version 1.0.

This page describes research, not your hair. Talk to your own doctor about your situation.

What we can say today

Researchers do not yet know how much hormones drive the hair loss many women notice around menopause. Some types of hair loss appear more often after menopause, but how hormones drive them is not settled. Data on hormone therapy for hair remain limited and inconsistent.

Why it matters

Thinning hair is a change many people find hard to live with, and falling estrogen often takes the blame. If that link were clear, research could focus on it. If other factors matter as much or more, the picture changes.

For research, hair loss is several conditions under one name. Female pattern hair loss, shedding after a stress or illness (telogen effluvium) and frontal fibrosing alopecia, a scarring type, behave differently. Studies that mix them make the hormone question harder to answer.

What is known

  • A 2026 systematic review in the American Journal of Clinical Dermatology found female pattern hair loss and frontal fibrosing alopecia had the strongest links to the postmenopausal period. Data on menopausal hormone therapy were limited and inconsistent, and the authors call for longitudinal studies. [1]
  • A 2025 review in Maturitas notes that female pattern hair loss, telogen effluvium and frontal fibrosing alopecia have been seen more often after menopause, and describes possible hormonal mechanisms. [2]
  • A 2026 review in Climacteric states that the cause of female androgenetic alopecia is unclear, that studies have failed to link testosterone levels to how common it is, and that obesity and insulin resistance may play a role. [3]
  • A 2022 cross-sectional study in Menopause of 178 postmenopausal Thai women found female pattern hair loss in 52.2%. After adjustment, only a body mass index of 25 or more was associated with it. [4]
  • A 2026 review in Dermatology states that hormone therapy is not currently indicated for hair loss alone. [5]

Why it is still open

Most of the evidence is reviews and cross-sectional studies. A snapshot can show that two things occur together, not which came first. Hormone levels in blood have not tracked hair loss well, and the factors that did show up, such as body weight, are linked to many other things. Hair loss types are often studied together. Studies of hormone therapy for hair are few and point in different directions.

A further 2026 systematic review on estrogen therapy and hair loss was published as a research letter without an abstract. We will describe its findings once we have read the full text.

What would answer it

A long term cohort that follows women through the menopause transition, records hormone levels, body weight and metabolic health, and classifies hair loss by type with standard measures. For the treatment part, randomized trials in clearly defined types of hair loss, with hair counts or photos as outcomes and follow-up of a year or more.
